otherwise require the level of care available
in an ICF/IID
Target population: Individuals with a diagnosis of Intellectual
Disabilities (ID); Individuals meeting the
Intermediate Care Facility for Individuals
with Intellectual Disabilities (ICF/IID) Level
of Care; Persons not residing in a group
home setting or environment; Persons
currently on the waiting list for ID services.
Provide:
Day Habilitation Services-Level 1-4
Day-Habilitation Services with Transportation – Level 1-4
Community Day Habilitation – Level 1-4
Community Day Habilitation with
Transportation – Level 1-4
Prevocational Services
Supported Employment Services
Individual Assessment/Discovery
Individual Job Coach
